Utilidad diagnóstica del SOFA modificado obstétrico (omqSOFA) frente al qSOFA y otros criterios en el diagnóstico de sepsis materna: revisión sistemática

Resumen

La sepsis materna es la tercera causa de mortalidad materna a nivel mundial, con una incidencia acumulada estimada de 13,16 casos por cada 10 000 embarazos y cerca de 17 670 muertes anuales atribuibles a sepsis e infecciones maternas. Los cambios fisiológicos propios del embarazo alteran los signos vitales que son usados por herramientas de cribado como el qSOFA, lo que ha motivado el desarrollo de versiones adaptadas para la población obstétrica, surgiendo así herramientas como el omqSOFA, qSOFA-P, omSOFA u omSIRS.  Esta revisión analiza la utilidad diagnóstica de estos criterios frente al qSOFA convencional y otras escalas (SIRS, MEWS) para predecir resultados maternos adversos severos, razón por la cual, se realizó una Revisión Sistemática de la Literatura siguiendo PRISMA 2020, con búsqueda en PubMed, Scopus y LILACS; de 413 registros iniciales, 10 estudios cumplieron los criterios de elegibilidad.  Los resultados muestran un rendimiento heterogéneo, debido a que, en cohortes institucionales específicas el omqSOFA y sus variantes superaron al qSOFA estándar (AUROC de 0,77 frente a 0,54; sensibilidad de 79% frente a 38%; especificidad de 91,3% frente a 86%), mientras que en poblaciones amplias y multinacionales su sensibilidad se redujo exponencialmente (18% a 38%), a expensas de una especificidad elevada (90 a 100%). En el qSOFA estándar, si no se le realiza algún ajuste, muestra un desempeño cercano al azar al presentar un AUROC cercano al 0,50-0,54.  Se demostró que la precisión diagnóstica del omqSOFA es mayor a la del qSOFA convencional si la comparamos con otros criterios de riesgo de sepsis, los cuáles son utilizados en otras poblaciones no obstétricas. Esta ventaja no persiste en las poblaciones heterogéneas, motivo por el que su utilidad está inmerso en protocolos institucionales que pertenecen a alerta materna a nivel hospitalario, mas no en su uso como criterio diagnóstico.
